Feedback Submitter Application is broken
When trying to sign up as a Feedback Submitter, the application does not recognize the "Yes" option for either "Do you own an Amazon Alexa device?" or "Have you built an Alexa skill in the past?" When trying to submit the application with "Yes" selected, it returns the error, "This field is required. Please enter a value." It does not return that error when selecting "No"; however, owning an Alexa device is required, so the application as-is is impossible to submit.
Tested on Brave, Firefox, and Chromium -- all three gave the same error.

For the two yes/no questions, it is kicking a "you must fill this in" error. It forces you to pick one, so it is never empty.

Hey Guys! I am not sure why it's not working because it seems to be working for me, but in an effort to override the issues I made those two fields optional. We still do need a response to those, though. Let me know if it works for you now.

Another weird issue -- after I submitted the reviewer form, I received an empty email with "Skill Feedback Submitter Application" in the title from "no-reply@wufoo.com" (guessing it shows as empty because of the CSS inside it or something?).
I was curious as to what it was, so I viewed the message source, and there's a surveymonkey link inside it...and THAT goes to a surveymonkey.com link that's a blank page, and if you change http to https, it's got a "sparkpost.com" SSL certificate installed, not surveymonkey...
Anyone else get this or know what's up w/it?

This is so strange. I'm going to reach out to Wufoo to look into it. It does seem like it's working again because I've seen many responses come through, but I've never had these problems using their forms before so time to ask the experts.

Hi all - I found out what caused the problem, in case anyone is interested. The dropdown needed to have a base input (like "Select one") in addition to the Yes/No in order to validate that you did select a choice. Learned something new today!
